The error appeared not to have been identified by those responsible for preparing or reviewing the submissions, nor by counsel, but was instead brought to light by the opposing party. The CPS had explained the matter as arising from the use of artificial intelligence combined with a failure of proper verification, and has taken steps internally to investigate, review and mitigate the risk of repetition.’
